If you test positive for COVID-19, notify our office immediately.  We have treatment options. These new treatments, are shown to reduce hospital admissions by 70%!

To qualify for treatment you must have 1) tested positive for COVID-19 and 2) must begin treatment within 7-days of first having symptoms. Additional criteria must be met, depending on age and health conditions.
If you get tested for COVID-19,
  • Tell the testing center to send a copy of your results to your provider at Premier Family Medicine.
  • Call us as soon as you receive your test results to discuss treatment options.

16+ Register & Schedule for the COVID-19 Vaccine provided by the State of Indiana

Hoosiers meeting the age requirement listed above can register, or be registered by a caregiver, for the free Indiana provided COVID-19 vaccine. Every county has at least one vaccine clinic and all registrants are asked to sign-up at a clinic near them. To register and schedule your time at an Indiana designated clinic near you, you have 2 options:

  2. Call 211 and register over the phone.

    2. If you want an earlier day / time than available at your clinic of first choice, consider looking at other clinics and possibly driving a little further.
The appointment time for the 2nd vaccine dose will be made at the clinic when you get your 1st dose.

This is a government sponsored program, your vaccine will be administered at a state designated clinic. The CDC recommends that patients at a higher-risk get the vaccine when it becomes available to them. Click below to determine if you are at a higher-risk.
